Traveling South on I-25 take Exit 96/Minnequa Ave. Go West on Minnequa Ave., our office will be on your left. Hospital on your right.

 

Traveling North on I-25 take Exit 96/Indiana Ave. Go West on Indiana Ave., Take a right on Orman Ave., then left on to Minnequa Ave. Our office will
be on your left. Hospital on your right.

We are located in the medical office building attached to the parking garage across the street From Parkview Medical Center. Parking is conveniently located in the parking garage. Follow sign to the entrance and go to the 2nd floor.

All 6 physicians have privileges at both St. Mary Corwin Medical Center and Parkview Medical Center in Pueblo.

Insurance

We are currently accepting most insurance plans. Please feel free to call our office to verify insurance participation.

It is very important that you have a current copy of your insurance card with you at all visits. Currently, well woman care, does not require a referral as long as you doctors are participating with your plan. If you have questions as to whether your particular plan requires a referral for anything other than well woman, please call the Customer Service number listed on your card. Please note that even though you don't need a referral for a well woman exam your insurance may or may not pay for services depending on your benefit package. Please call us and our office staff will
be happy to help you.

Emergencies

24-Hour Coverage. In the event of an emergency, there is a doctor available 24-hours a day. After office hours, your call will be routed to the doctor on call. If you have a life threatening emergency, call 911. For non-emergent concerns, please call our office during regular business hours. the doctors and nurses will return your call as soon as possible. Unless you have an urgent issue, the majority of phone calls are returned at the end of the morning and at the end of the afternoon. Please leave numbers where you can be reached during those times.

Prescription Refills

Please do not wait until your medication runs out before calling for a refill! If you need a refill of a current prescription, please have your pharmacy fax us a refill request. Plan to allow 72 hours for a prescription to be renewed. We may request that you have an office appointment to ensure that there are no new problems and the medication is working properly.

Financial Policy

All payments are due at the time treatment is rendered unless prior arrangements have been made. As a courtesy, we file all applicable insurance claims for you. However, if your insurance denies payment on services you are responsible for all charges. You agree to pay all reasonable rebilling fees, attorney fees, and court costs associated with the collection of any debts as allowed by Colorado State Law.

We are very aware of today's healthcare costs and make every effort to keep our fees fair and reasonable.

Since every employer (and their insurance coverage) is unique, we cannot be knowledgeable on what services are covered (or not covered) and what referrals are required. It is YOUR responsibility to find out this information BEFORE you visit.

Self Pay Patients

Regrettably, we have limited appointments for patients that have no insurance. However, if we make an appointment for a patient who does not have ANY insurance, then we will grant a 20% discount on certain services when paid in full on or before the date of service.

Surgery Prepayment

Prior to surgery or a special procedure being done, our staff will call your insurance company to verify eligibility and benefits. If you have not met your full deductible or if your insurance company requires that you are responsible for a coinsurance, we will ask for a prepayement prior to rendering the service. Someone from our billing department will explain the calculation of the prepayment amount and will assist you in setting up a payment plan for the remaining balance due.

Obstetrical Services

If you have no insurance, you will be asked to pay a $350.00 payment on your first visit with us. Self pay and insurance patients must bay the balance of the total OB fee estimate prior to delivery. Our staff will call your insurance company to verify eligibility and benefits and send you an OB payment plan; our experience is that this method works best for both parties.

Sterilization and Contraceptive Devices

Many insurance companies do not cover these procedures. If your policy does not cover contraception, you will be asked to pay the entire fee prior to the surgery being booked or the insertion of the contraceptive device.

Co-Pays

Co-pays are to be paid at the time the service is rendered. It is required by your insurance carrier that we collect co-pays since it is calculated in the insurance carrier's total payment to us.

*For your convenience we accept Visa, MasterCard, AMEX, Discover and debit cards.

*Any pathology or laboratory that you may have will be billed by the pathology or laboratory company themselves. any questions you may have with these bills should be directed to the company that performed the test.
