| Our office is open Monday through Friday 8:30am - 5:00pm. Please call during business hours for all appointments and prescription refills. Our friendly staff will be happy to assist you. We are located in the medical office building attached to the parking garage, across the street from Parkview Medical Center. Parking is conveniently located in the parking garage. Follow sign to the entrance and go to the second floor. |

Traveling south on I-25 take the 13th St. Exit. Go West on 13th. Take a right on Grand Ave. Office and parking garage will be on your right; hospital will be on your left. Turn right into the parking garage. Traveling north on I-25 take the 13 th St. Exit. Go West on 13 th. Take a right on Grand Ave. Office and parking garage will be on your right; hospital will be on your left. Turn right into the parking garage. |
